McDonough County Quality of Life Youth Task Force is Taking Applications for Youth Awards

The McDonough County Quality of Life Youth Task Force is pleased to announce our annual awards program to recognize youth residing in McDonough County. Youth will be recognized for “their actions in the service of others demonstrating their commitment to improving the quality of life for residents of McDonough County.”   Nominations are now being accepted from residents, organizations, school officials, teachers and school staff for school aged children.   The public is encouraged to either send their nomination to the Regional Office of Education by Friday, March 29, 2019 or use this online format by using the following link: https://goo.gl/forms/j9ClaJEsu0Zjn3DG3

 

If you do not choose the on-line nomination format, nominations should be legible or typed and limited to a single sheet of paper.   Contact information should include name, address and telephone number of nominator for verification.   We would appreciate no family nominations.

 

The Youth Awards will be presented at the local School District Awards Assemblies this spring if applicable.
